(DCTRC) developed Internal Care and Medical Coordination in Group Homes, a practical training built for medical coordinators, direct care workers, and program administrators.
This course was developed as part of DCTRC’s continued commitment to Michigan’s specialized group homes, giving staff a structured framework for the day-to-day work of coordination: understanding the group home environment and a coordinator’s responsibilities, building effective partnerships with physicians, nurses, pharmacists, and case managers, and learning to recognize and document meaningful changes in a resident’s health.

The course also covers prescription management — tracking medications, coordinating refills, and educating residents — and closes with strategies for stronger internal communication, from building effective care plans to running productive team meetings. Together, these five chapters give group home teams a repeatable process for keeping medical information organized and residents safely supported.
